Signal Boost: In brief

  • We measured the impact of personalized engagement across a five-year study.
  • Completed clinical actions increased from 34% before launch to 53% in Year 5, representing a 57% lift.
  • Annual physical completion rose from 21% to 53%; colorectal cancer screening rose from 29% to 57%.
  • Most measures reached their highest completion rates in Year 5, signaling that sustained personalization can drive lasting behavior change.

Benefits leaders often measure engagement by opens and clicks. But the more consequential question is whether employees actually complete the annual physical, follow through on the cancer screening, or take the steps needed to manage an ongoing condition.

And beyond that first action, does the impact keep building over time?

That’s where Evive offers a useful signal.

Evive is bswift’s personalized recommendation engine. It delivers year-round, data-driven nudges designed to guide employees toward cost-conscious decisions and healthier outcomes. Each logic-backed recommendation triggers timely, relevant messages that drive action, whether encouraging preventive care, supporting condition management, or improving benefits utilization.

Evive draws on millions of data points, including claims and spending behavior, enrollment and eligibility data, employee-selected preferences, and patterns across employee interactions over time. Together, these inputs help Evive personalize engagement with greater precision, making each interaction more timely, relevant, and better aligned to individual needs.

For this analysis, the Signals by Evive data team examined completion rates across 13 HEDIS-aligned clinical measures and approximately 482,000 eligible member-actions. The multi-client analysis included four clients with multiple years of tenure, with measurement periods varying between 2018 and 2024.

The pre-launch measurement year served as the baseline. The latest available results reflect Year 5, while Evive deployment remains ongoing. The findings provide a population-weighted view of how completion rates changed following the introduction of Evive engagement. Results may vary based on population, benefit design, and communication strategy.

Personalized engagement drove a 57% lift in completed actions

Across the full measure set, the population-weighted completion rate increased from 34% before Evive launched to 53% in Year 5.

That represents:

  • A 19.3-percentage-point absolute increase
  • A 57% relative lift in completion rates
  • Improvement sustained across five years and counting

Most measures reached their highest completion rates in the latest measurement year. The effect did not peak immediately after launch and then fade. It continued building over time.

The aggregate picture: Lift across the full measure set

Across the full 13-measure portfolio and ~482,000 eligible member-actions, Evive engagement produced a population-weighted lift that is both clinically meaningful and operationally durable.

Evive improved completion across preventive visits, cancer screenings, and chronic-condition management. The gains included actions with substantial room for improvement as well as measures that already had high completion rates before launch.

Annual physical completion more than doubled

Annual physical completion increased from 21% at baseline to 53% in Year 5, a gain of 32 percentage points and a 152% relative lift across approximately 88,000 eligible members.

Annual physicals can serve as an entry point to many other preventive actions. They create opportunities to identify emerging health needs, recommend appropriate screenings, and connect members with additional care.

By helping more employees complete routine visits, personalized engagement can support the next set of decisions that follow.

Colorectal cancer screening nearly doubled

Colorectal cancer screening increased from 29% to 57%, representing a 28-point increase and a 97% relative lift across approximately 19,000 eligible members.

Screening recommendations can be easy to overlook when they arrive as part of a broad preventive-care campaign. A personalized recommendation makes the action individual: this screening applies to you, and this is what to do next.

That relevance may be particularly important for actions that employees understand in principle but continue to postpone.

Blood pressure management improved from an already-high baseline

Blood pressure management rose from 77% to 87% — a 13% relative lift on a measure that was already considered high-performing. When most of a population is already engaged, the remaining gap is typically the hardest to close, which makes this movement notable.

The most meaningful lift happened after Year 1

The analysis found measurable lift across nearly every tracked measure during the first measurement cycle. In subsequent years, the impact continued expanding. By Years 4 and 5, completion rates remained sustained or continued rising across high-impact areas including preventive visits, cancer screenings, and chronic care.

Most measures reached their highest completion rates in Year 5, the latest measurement period available. Blood pressure management, cholesterol management, breast cancer screening, and cervical cancer screening all peaked in that year. Colorectal cancer screening continued rising from 29% before launch to 57% in Year 5.

That distinction is especially relevant because benefits engagement frequently produces its greatest visibility at launch. A new program receives concentrated promotion, employees encounter it for the first time, and initial activity rises.

A launch-week lift is relatively easy to produce and doesn’t say much on its own. The more useful question for evaluating any engagement program is what its numbers look like in year three, once the initial promotional push is long over.

Personalized engagement can remain useful because the recommendation does not have to remain static. As an employee’s eligibility, health activity, spending, and interactions change, the next relevant action can change too.

Bottom line: the goal is to continue identifying the action most relevant to that member at that moment.

What this means for benefits engagement

A broad campaign can establish that an action matters. Personalization makes clear whether it matters to a specific employee, why it matters now, and what to do next.

That relevance helps turn attention into completed action. Instead of sending the same message to everyone, Evive connects members with the preventive care, screening, or condition-management step most relevant to their circumstances.

Across an average company with 10,000 employees, that can translate to approximately 4,400 incremental clinical actions completed after Year 1.

Those aren’t abstract gains. A blood pressure management rate moving from 77% to 87%, or a colorectal screening rate that nearly doubled, are the kinds of shifts that tend to show up later in stop-loss claims and utilization data (not just in an engagement dashboard). More completed screenings and better-managed chronic conditions generally mean earlier detection and fewer avoidable ER visits down the line. That’s a different, more durable kind of evidence than an open rate.

The broader pattern worth sitting with: most benefits engagement programs show their strongest numbers in the first year, then flatten as novelty wears off. This dataset points to a different possibility: with the right design, engagement can keep compounding well past year one, on exactly the measures that are hardest to move.

Methodology: Pre-launch baseline is calculated as the measurement year immediately prior to Evive launch. Latest measured rate reflects Year 5, the most recent measurement period available; Evive deployment is ongoing. Results are based on a multi-client study of four clients, with measurement periods varying by client between 2018 and 2024. Aggregate figures are population-weighted across ~482,000 eligible measure-instances spanning 13 HEDIS-aligned clinical actions. Results may vary by population, benefit design, and communication strategy.
